Developing Innovative Community-Based Long-Term Care Systems and Services

<p>Showcasing innovative community-based long-term care (LTC) models in Indonesia Mongolia and Viet Nam this publication looks at ways integrated and holistic care and support services can underpin aging in place and improve health outcomes.</p><p></p><p>The publication highlights policy changes the three countries have made to scale up cost-effective community-based LTC. It shows how increasing financing and legal frameworks and harnessing technology for digital care and case management can help support caregivers and build the inclusive and integrated systems needed for rapidly aging populations.</p>
