In this context the required forms of healthcare and medical services are also evolving. Integrated care which seamlessly connects medical institutions nursing facilities local communities and the homes of patients is becoming increasingly vital. Medical IoT utilising advanced technologies such as wearable devices is expected to progress further and play a critical role in the success of such care.</span></p><p><span style=color: rgba(23 43 77 1)>Many older individuals suffer from multiple chronic conditions making collaboration among healthcare professionals across various specialities essential. Efficient cooperation between various professions and institutions along with the swift and appropriate sharing of essential information is highly effective in many areas including patient safety the effective delivery of care and ultimately improving patient and family experience.</span></p><p><span style=color: rgba(23 43 77 1)>Older individuals are vulnerable both physically and socially and it is vital for society as a whole to support them.
